Presented annually, the Dr. Timothy C. Frewen Humanitarian Award recognizes individuals and teams that make a difference at Children’s Hospital at London Health Sciences Centre, and St. Joseph’s Health Care London, Neonatal Intensive Care Unit. The award distinguishes and honours those who exemplify the qualities of outstanding compassion and humanitarianism in the provision of child and family care.
Any health care professional, support staff, volunteer or student/resident working at Children’s Hospital or at St. Joseph’s Health Care London, Neonatal Intensive Care Unit in 2008 is eligible. The nominator may include a group of individuals they recognize as a team as collectively exemplifying the qualities of the award. Previous nominees may be nominated again.
Each nominee should exemplify humanitarianism both in their influence on patient care, and also through the respect, kindness and compassion with which they treat and communicate with families and colleagues. Detailed selection criteria and nomination guidelines can be found on the Nomination Form.
Please download, print, and complete an official Nomination Form. Along with the form please include 1 to 2 pages summarizing why you feel the individual deserves the Dr. Timothy C. Frewen Humanitarian Award. (Be sure to give specific examples of how the nominee has met the selection criteria.)
Submit the Nomination Form and accompanying letter to Children’s Health Foundation no later than 5:00pm on Friday, October 3, 2008.
The award will be presented on November 13, 2008, at a special award luncheon held in the 7th Floor Amphitheatre, Westminster Tower, Victoria Hospital.
